500 Server Error on first application

hi all,
I'm a new user and am having a problem with my first app. Hopefully someone can help.

1. I successfully installed Dataface. I successfully see the dataface_info.php message
2. I have successfully generated a test app with makesite
3. when trying to display the test app I get a "500 internal server error" message (Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.)

apache error logs show:
Premature end of script headers: /home/myuser/public_html/butacadb/index.php
File does not exist: /home/myuser/public_html/500.shtml

I'm fairly certain it is a permission/ ownership configuration with mysql. I'll fix and retry.

Not a permissions error from what I can tell.
